I am traveling on the westbound cruise in October and wondering whether I should extend my stay in Prague and if so, for how long? Thanks

First, skip the Regensburg excursion and take the monastery excursion instead. It is one of the few chance to actually go into the library rooms at the monastery rather than looking in from the door. I would give yourself at least 2 additional full days in Prague. The castle town area even with skipping the monastery is at least a 1/2 day tour. Then you have the old town, lesser town and new town areas. There are areas outside of Prague that are worth seeing on a day trip. You could even go to Cesky Krumlov from Prague rather than during your river cruise.
It's pretty much always worth a pre and/or post cruise stay if you can afford it. Most cruises start/end in interesting cities with lots to do. Sadly most people don't start thinking about that until they've already booked air or it's too late to add days at the Tauck hotel.
