I was thinking that during free time, it might be fun to treat myself to a massage or pedicure. I noticed that Amboseli offers treatments, and maybe the others, too. Do these have to be booked in advance?

Not necessarily. But do be sure you book giving you time incase the safari ends later than expected. This can happen quite often if exciting things are happening out on safari.
I did have a massage there. Booked it and a horseback ride shortly after arrival. The staff were.set-up in a common area, where we made the bookings.
