Questions for recent travelers to Tanzania and Kenya:

Tanzania evisa website says US Passport holders must apply for a "multiple entry" visa, not an ordinary visa.

Also, how did you complete and print the Traveller Health Form required to be submitted within 24 hours of arrival if you were enroute?

Any assistance appreciated.
