First, skip the Regensburg excursion and take the monastery excursion instead. It is one of the few chance to actually go into the library rooms at the monastery rather than looking in from the door. I would give yourself at least 2 additional full days in Prague. The castle town area even with skipping the monastery is at least a 1/2 day tour. Then you have the old town, lesser town and new town areas. There are areas outside of Prague that are worth seeing on a day trip. You could even go to Cesky Krumlov from Prague rather than during your river cruise.
It's pretty much always worth a pre and/or post cruise stay if you can afford it. Most cruises start/end in interesting cities with lots to do. Sadly most people don't start thinking about that until they've already booked air or it's too late to add days at the Tauck hotel.
You can never go wrong with Prague. I've been there three times. I recommend taking some guided tours that you can book on sites like Viator and Get Your Guide. Prague Castle isn't one building; it's a whole district. On a walking tour, the guide pointed out the window that was the setting for the Defenestration of Prague, one of the proximate causes for the 30 Years' War. If I had been touring on my own, I would have walked right by it. After a walking tour, go back and wander the castle district by yourself. Also wander around Mala Strana, and see the John Lennon Wall. A couple of my favorite walking tours were one of Josefov, the Jewish district, and one titled something like "Prague during WW II." Dan Brown's latest book, "Secret of Secrets" is set in Prague. When I was there this past fall, I saw displays of the book both in English and in Czech. You might want to read it before you go.
